To achieve the perfect 45° angle, ask the patient to hold a pen or foam wedge between the fingers. This provides stability and the exact required rotation.

Patient Positioning

Place the hand in prone position on the image receptor.
Rotate the finger laterally 45° (away from the thumb).
Extend the finger being examined and separate it from the others.
Use a 45° foam wedge to maintain stability and avoid motion.

Location: Perpendicular to the proximal interphalangeal (PIP) joint of the affected finger.

Visible Anatomy

Phalanges

Distal, middle, and proximal phalanges visible.

Joints

Open DIP and PIP joint spaces.

Metacarpal
